Tapentadol – Uses, Dosage & Side Effects

What is Tapentadol?

Tapentadol is a centrally-acting analgesic with a dual mechanism combining mu-opioid receptor agonism with norepinephrine reuptake inhibition (MOR-NRI). Marketed as Nucynta, it provides potent pain relief often with fewer gastrointestinal side effects compared to traditional opioids.

Uses and Benefits

  • Acute moderate to severe pain
  • Chronic severe pain requiring daily opioid treatment (ER form)
  • Neuropathic pain, particularly diabetic peripheral neuropathy (FDA-approved for ER)

Dosage Information

Immediate-Release (50mg, 75mg, 100mg): 50-100mg every 4-6 hours. Max: 700mg Day 1, then 600mg/day.

Extended-Release (50-250mg): Start 50mg twice daily. Max: 500mg/day.

Side Effects

Common: Nausea, dizziness, somnolence, constipation, headache

Serious: Respiratory depression, serotonin syndrome, adrenal insufficiency, seizures

Precautions

  • Contraindicated with significant respiratory depression
  • Do not use with MAOIs or within 14 days of stopping an MAOI
  • Avoid alcohol and CNS depressants
  • ER tablets must be swallowed whole
  • Not recommended in severe hepatic impairment
How is Tapentadol different from Tramadol?
Tapentadol is more potent (Schedule II vs IV), has stronger opioid binding and NRI, and is FDA-approved for neuropathic pain. Generally causes less nausea and constipation.
Is Tapentadol suitable for neuropathic pain?
Yes. Tapentadol ER is specifically FDA-approved for diabetic peripheral neuropathy pain.
